Information for Clients and Families

The National Disability Insurance Scheme (NDIS) is rolling out across New South Wales from 1 July 2016 to 30 June 2018. To help prepare for the roll out some of your information will be shared with the National Disability Insurance Agency (NDIA).

What information will be shared?

Only necessary information will be shared with the NDIA. This information may already be held by Family and Community Services (FACS) or will be collected from your service provider. Your basic identifying information will be used, such as:

  • name
  • date of birth
  • preferred language spoken at home
  • contact details
  • carer contact details
  • types of supports received.

Why will my information be shared?

Sharing this information will assist in determining whether you automatically meet the disability requirement for access to the Scheme and will ensure that the NDIA knows the best way to contact you to commence your planning process when the time comes to transition.

How will my information be used?

  • your information will be held securely by FACS
  • your information will only be shared with the NDIA
